Let’s succeed and make a difference, together! A well known Butte County medical group is seeking a dedicated Case Manager (LVN) to join their care team! Position Overview: - Provide comprehensive case management and care coordination for patients, ensuring they receive the medical, behavioral health, and community-based support they need. Key Duties/Responsibilities: - Coordinate care, referrals, and community resources - Facilitate authorizations for supporting medications - Collaborate with behavioral health clinicians and licensed medical staff - Assist with referrals for crisis support and counseling services - Support care-related documentation, including medical and legal coordination - Advocate for patients and connect them with appropriate services - Develop and maintain referral and resource lists Requirements: - Active LVN License - BLS Certification - Case management or care coordination experience preferred - Experience with prior authorizations a plus - Bachelor’s Degree in Social Work or 2+ years of relevant case management -experience preferred - Experience working with the LGBTQ+ community a plus Schedule: - Monday – Friday | 8:00am – 5:00pm - Full time | 40 hours/week Wage: -$ 30.05/hour to start!
