Following up on a previous post—I wanted to highlight the impressive change we saw after just one set of a low-threshold hip flexion isometric: The patient initially had a significant limitation in hip flexion ROM (struggling to reach 90 degrees) and restricted Internal Rotation (IR). After performing the isometric holds: Improved Hip Flexion: We were able to passively achieve above 90 degrees of hip flexion with much less effort (as shown in the video). Improved Hip Internal Rotation (IR): This secondary finding is huge! Improving IR is often a major goal in hip rehab, and seeing it change so rapidly suggests a powerful neurophysiological effect. Changes in hip IR and flexion are huge for closed chain loading. This reinforces the concept that what feels like a structural "block" or "tightness" is often a motor control inhibition or a protective pattern related to a lack of local muscle capacity or simple how I put it - inability to load. By using a simple, low-threshold isometric, we are: Modulating pain by providing a strong, non-threatening input. Re-educating the motor program for the hip flexors, which subsequently reduces protective tension/guarding in the joint capsule and surrounding muscles. The take-home: Don't just stretch! Using isometrics to improve the capacity of the muscles around the hip—even in a seemingly "tight" joint—can be your fastest route to increasing both flexion and IR ROM and improving functional movements like squatting!
