Let's be honest, if you're still relying solely on the 0-5 Manual Muscle Test scale for anything beyond a basic neurological screen, you're leaving a mountain of valuable data on the table. It's subjective, has poor reliability for grades 4 and 5, and frankly, our patients deserve better. Huge props to everyone who has already made the switch to objective strength testing with a handheld dynamometer (HHD). 👏 You're already ahead of the curve, getting real numbers to track progress and demonstrate value. But are you getting the full picture? We often get fixated on peak force and limb symmetry indexes. "The involved side is 20% weaker in peak isometric force." That's a great start, but it's not the end of the story. Let's level up the conversation. 🚀 I'm talking about Rate of Force Development (RFD). RFD measures how quickly an individual can generate force. It's essentially explosive strength, calculated as the change in force over the change in time (ΔF/Δt). Think about its functional relevance: ✅ Preventing a fall after a stumble ✅ A soccer player's first step to the ball ✅ An ACL-R athlete's ability to "put the brakes on" during a cutting maneuver An athlete might have perfectly symmetrical peak force, but if their RFD is significantly slower on their surgical side, you're looking at a potential performance deficit and an injury risk hiding in plain sight. So, for all the clinicians out there already using HHDs: Are you measuring Rate of Force Development in addition to peak force? If so, what are you seeing? If not, what are the barriers?
