“Headache” มี step ในการ approach ยังไงบ้าง . 🟦ก่อนอื่นเราต้องรู้ก่อนว่า แบ่งเป็น 2 กลุ่ม . ⚫️Primary headache คือ ปวดหัวที่ไม่มีสาเหตุชัดเจน และไม่รุนแรงถึงชีวิต . ⚫️Secondary headache คือ ปวดหัวที่มีสาเหตุที่ชัดเจน เช่น SAH, Brain tumor ซึ่งมีความอันตรายมากกว่า และถึงขั้นรุนแรงถึงชีวิตได้ดังนั้น . 🟦STEP 1 : เราจึงต้องพยายามหา Secondary headache ก่อนเสมอ โดยดู Red flag signs ดังต่อไปนี้ . S N O O P4 . ⚫️Systemic symptoms (fever, wt. loss) หรือ Secondary risk factor(HIV, cancer) ⚫️Neurological deficits ⚫️Onset sudden ⚫️Older age at onset (> 50 years) ⚫️Pattern changes ⚫️Positional headache ⚫️Precipitated by valsalva or exertion ⚫️Papilledema & signs of ↑ICP . ====================== . 🟦STEP 2 . ⚫️ถ้ามี Red flag signs เราจะนึกถึง Secondary headache ซึ่งก็ควร Work up หา Intracranial cause ตามสาเหตุที่นึกถึงต่อไป . เช่น สงสัย SAH ก็ไป CT brain, สงสัย Meningitis ก็ LP . ⚫️แต่ถ้าไม่มี Red flag signs ก้จะมาตกในกลุ่ม Primary headache ซึ่งก็ต้องไปดูว่า เข้าได้กับ Pattern ของโรคไหน . ====================== . 🟦STEP 3 : ทีนี้เรามาลองดูว่า Primary และ Secondary headache มีโรคอะไรได้บ้าง . ⚫️Primary headache . ▪️ Migraine headache ▪️ Tension-type headache ▪️ Cluster headache . ⚫️Secondary headache . ▪️ Infection : meningitis, encephalitis, brain abscess . ▪️ Trauma : epidural hemorrhage(EDH), subdural hemorrhage(SDH), intracerebral hemorrhage(ICH) . ▪️ Tumor : primary brain tumor, metastatic brain tumor . ▪️ Vascular : subarachnoid hemorrhage(SAH), stroke, cerebral venous thrombosis . ▪️ Autoimmune : Giant cell arteritis . ▪️ Drug/ toxin : alcohol, CO poisoning, Medication overuse headache, caffeine withdrawal headache . ▪️ Other : hypertensive encephalopathy, trigeminal neuralgia, idiopathic intracranial hypertension, hydrocephalus, post-LP headache, glaucoma, sinusitis, preclampsia . ==================== . 🟦ลักษณะของ Primary headache ทั้ง 3 แบบ . 🔵Migraine headache . ▪️ Character : Unilateral > bilateral, throbbing ▪️ Duration : 4-72 hr. ▪️ Intensity : Moderate to severe ▪️ Additional symptom : N/V, Photophobia, Phonophobia, อาจมี Aura นำมาก่อน ▪️ Trigger : Stress, hormone fluctuation(OCP, mense), อาหารหมักดอง processed meat แฮม ไส้กรอก ชีส chocolate . 🔵Tension-type headache . ▪️ Character : Bilateral, dull, band-like compression ▪️ Duration : 30 min - 7 days ▪️ Intensity : Mild to moderate ▪️ Additional symptom : No ▪️ Trigger : Stress, anxiety, depression, lack of sleep, fatigue . 🔵Cluster headache . ▪️ Character : unilateral, periorbital, piercing, stabbing, burning ▪️ Duration : 15-180 min, ปวดถี่ตั้งแต่วันเว้นวัน - 8 ครั้ง/วัน ▪️ Intensity : Severe ▪️ Additional symptom : autonomic symptoms(lacrimation, rhinorrhea, sweating), Horner’s syndrome ▪️ Trigger : Alcohol . ==================== . 🟥สุดท้ายเราลองมาดูตัวอย่างโรคในกลุ่ม Secondary headache กันนะครับ 🔴Subarachnoid hemorrhage(SAH) ▪️ Hx : Sudden onset severe headache(thunderclap headache) ▪️ PE : Stiff neck positive 🔴Meningitis ▪️ Hx : ปวดศรีษะทั่วๆ คอแข็ง ไข้สูง ▪️ PE : Stiff neck positive 🔴Giant cell arteritis ▪️ Hx : ปวดบริเวณขมับ ปวดกรามเมื่อเคี้ยวหรือพูด(Jaw claudication) มองเห็นภาพเบลอ ภาพซ้อน ไข้ นน.ลด ▪️ PE : Scalp temporal artery tenderness 🔴Brain tumor หรือ Space-occupying lesion ▪️ Hx : ปวดศรีษะมากจนต้องตื่นขึ้นมาจากขณะที่หลับหรือเป็นมากในตอนเช้า อาการปวดเป็นมากขึ้นในหลักสัปดาห์หรือหลักเดือน คลื่นไส้อาเจียน ชัก (เบื่ออาหาร นน.ลด ในกรณี malignancy) ▪️ Risk : (โรคมะเร็ง ในกรณี malignancy) ▪️ PE : Signs of ↑ICP, neurodeficit 🔴Traumatic intracranial bleeding ▪️ Hx : ประวัติ recent trauma หมดสติ ซึมลง อาจตามมาด้วย lucid interval, คลื่นไส้อาเจียน ชัก ▪️ PE : Signs of ↑ICP, neurodeficit 🔴Venous sinus thrombosis ▪️ Hx : ปวดหัว ซึม ชัก คลื่นไส้อาเจียน เห็นภาพซ้อน ▪️ Risk : Hypercoagulable(OCP, pregnancy), cancer, ENT infection ▪️ PE : Signs of ↑ICP, neurodeficit, CN palsy 🔴Acute glaucoma ▪️ Hx : ปวดรอบกระบอกตารุนแรง ตามัว คลื่นไส้อาเจียน ▪️ PE : Mid-dilated pupil, ciliary injection, cloudy cornea 🔴Sinusitis ▪️ Hx : ปวดใบหน้าบริเวณโพรงอาการ ไข้ ไม่ได้กลิ่น ▪️ PE : Facial tenderness over the sinus, purulent rhinorrhea 🔴Trigeminal neuralgia ▪️ Hx : unilateral, stabbing pain ปวดแปล๊บ เหมือนไฟฟ้าช็อตบริเวณใบหน้า(ตามแขนงของ CN 5) ปวดมากขึ้นเมื่อเคี้ยวอาหาร พูด หรือสัมผัส 🔴Medication overuse headache ▪️ Hx : ประวัติ chronic headahe ร่วมกับประวัติทานยาแก้ปวดเยอะๆ . ส่วน Signs of ↑ICP มีอะไรบ้าง ใครยังจำไม่ได้ ลองดูใน comment ได้เลยนะครับ . ใครอ่านจบแล้วก็เม้นทักทายกันได้นะครับ หรือพิมพ์ “เข้าใจแล้ว Headache” สวัสดีครับ^^ #พี่ซีMedsalmon
